New patients — We are a referral-based practice. Your optometrist or family doctor can fax a referral directly to our clinic, and our intake team will contact you to schedule your first visit.

How to get referred
Uptown Eye Specialists is a referral-based ophthalmology practice. We do not accept walk-in appointments. Here’s how to start your care with us.
See your doctor
Visit your optometrist or family doctor and ask for a referral to Uptown Eye Specialists. They’ll fax the referral directly to our clinic.
We’ll contact you
Once we receive your referral, our intake team will call you to schedule your appointment at the most convenient location.
Your first visit
Arrive 15 minutes early with your health card. See our First Visit guide for full details. Bring your health card, photo ID, list of medications, and any previous eye test results from your optometrist.
